Hearth Connection Homeless Program

Bois Forte Homeless Program is looking for people who have long histories of homelessness who are interested in receiving and engaging in supportive services to help end their homeless situation.

Participants who qualify will work with the Bois Forte Homeless Program Case Manager to find and keep affordable housing and meet other personal goals addressing areas that have contributed to homelessness.

The caseworker will act as a referral source, advocate, and support person for the participant, helping connect the individual with area resources and programs that they are qualified to receive.

This program is primarily centered on providing Supportive Services to those individuals with long histories of homelessness utilizing a Harm Reduction, Housing First Services Model.

Housing subsidy vouchers are not guaranteed with program enrollment. Award is based on regional availability.

*Folks are encouraged to apply for all housing options available

Emergency Housing or Emergency Financial Assistance is not available to those not enrolled in the program.

Who qualifies for the program?

  • Head of household must meet the State’s long term homeless definition:
  • Homeless continuously for a year or more -OR- experienced 4 episodes of homelessness in the past 3 years
  • Be 18 years of age or older
  • Has completed a VI-SPADT assessment through the coordinated entry process.

*All program intake is taken off the coordinated entry waitlist.

Homeless means:

  • Living on the streets, in a shelter or other places not meant for human habitation
  • Living in an institution or transitional housing, but homeless before you entered, OR
  • “Couch hopping” or “doubling-up” with friends or family for limited periods of time, moving from one place to another.

The Homeless Program Case Manager can help assist in connecting you to other resources available, assist with filling out applications and answer any questions you may have.
