Photocopies will not be accepted.)<\/p>\n<ul>\n<li>Original certified birth certificate<\/li>\n<li>State Issued Driver\u2019s License<\/li>\n<li>\u00a0State Issued Identification Card<\/li>\n<li>Passport<\/li>\n<li>\u00a0For Replacement Cards and for any changes made before the expiration date, there will be a $12.00 fee (cash or check) *Exact amount.
