Reminder Regarding Bois Forte Address Updates
The Address update form is available on the Bois Forte Website. Click on the resource tab, then enrollment, go to the change of name or address tab and print form.
If you cannot print the form, please send a written note with your full name, date of birth, current address.
Both need to be dated and have the original signature.

Address changes received by fax, email, and/or phone calls are not acceptable and will not be updated.
PLEASE VERIFY WITH THE UNITED STATES POSTAL SERVICE THAT YOUR ADDRESS IS CORRECT AND YOU ARE ON THE LIST TO HAVE MAIL DELIVERED TO THE MAILING ADDRESS YOU ARE PROVIDING. IF YOU ARE NOT ON THE LISTING WITH YOUR LOCAL POST OFFICE YOUR MAIL WILL BE RETURNED TO SENDER.
