Enrollment – Update Your Address
UPDATE YOUR ADDRESS
All address updates are accepted only by mail or in person.
Address Update forms can be downloaded from the Bois Forte Webpage under the Enrollment Tab.
Band Members may submit an address update in writing to the Bois Forte Enrollment Office, please have the letter include the following information:
*Name
*Address
*DOB
*Signature
*Date
*any address updates missing this information will not be updated.
