Bois Forte Tribal Identification Cards

Individuals are required to wear a mask, but can remove to take a picture.
Temperatures will be taken at the door.
Only individuals with appointments will be allowed in the building as there will not be a waiting area.

Please bring one of the following to get a Tribal ID: (Original Documents. Photocopies will not be accepted.)

• Original certified birth certificate
• State Issued Driver’s License
•  State Issued Identification Card
• Passport
•  For Replacement Cards and for any changes made before the expiration date, there will be a $12.00 fee (cash or check) *Exact amount. We do not have change.
